Consideration of Submandibular (Undescended) Ectopic Parathyroid Glands in Surgery and Localization Studies Anatomyof theparathyroidglandsvaries.Duringembryogenesis, precursor glands descend from the third and fourth pharyngeal pouches into the neck, but up to 20% end up in “ectopic” locations.1 Adenomas that develop fromundescended glands in the submandibular region aredifficult to identify on standard imaging andaccount for 7%of cases of persistenthyperparathyroidism following parathyroidectomy.2 Undescendedadenomasarewell described inendocrine surgery literaturebutnearly absent fromotolaryngology literature. With approval fromEastern VirginiaMedical School’s institutional reviewboard to use patient cases in a deidentified format,we identified the senior authors’ (J.T.W. andD.W.K.) patientswith undescended parathyroid adenomas.

Parathyroid adenoma Localization

Background: Bilateral neck exploration is the gold standard for parathyroid adenoma localization in primary hyperparathyroidism. But surgeons do not have adequate experience for accurate surgical exploration and new methods are developed for surgery like unilateral exploration and minimally invasive surgery, thus, preoperative localization could reduces time and stress in surgical performance....

Significance of MIBI scintigraphy in a patient with hydatid cyst and parathyroid adenoma

A 17-year-old boy with hyperparathyroidism exhibited a large cyst in the left hemi-thorax on his chest X-ray. The cyst was identified as a hydatid cyst. He was referred for biphasic methoxy isobutyl isonitrile (MIBI) parathyroid scan. A discrete ectopic parathyroid was detected in the left upper mediastinum in addition to accumulation of the MIBI tracer in the cyst wall. Using radio-guided occu...

SPECT-CT fusion in the diagnosis of hyperparathyroidism

Objective(s): In this study, we aimed to analyze the relationship between the diagnostic ability of fused single photon emission computed tomography/computed tomography (SPECT/CT) images in localization of parathyroid lesions and the size of adenomas or hyperplastic glands. Methods: Five patients with primary hyperparathyroidism (PHPT) and 4 patients with secondary hyperparathyroidism (SHPT) we...

Association of Hearing Impairment and Mortality in the National Health and Nutrition Examination Survey.

Discussion | As illustrated by these cases, undescended parathyroid adenomas are challenging to identify with sestamibi studies. When sestamibi studies do not demonstrate uptake in the thyroid bed or mediastinum, careful scrutiny for asymmetry of the submandibular regions is warranted to rule out these lesions. Alternatively, 4-dimensional CT imaging is a useful adjunct for localizing undescend...

Localization of ectopic parathyroid adenoma with Tc-99m-MIBI [Persian]

Ectopic paratliyroid adenoma might create a clinical dilemma in patients with hypercalcemia. Radioisotopic scan is a sensitive and noninvasive method which can be useful in these patients. In this report a case of ectopic parathyroid adenoma is presented and the usefulness of TC-99m-MIBI scintigraphy as compared with other modalities is discussed.
